The AI Agentic Sanatorium exists to bridge that gap by making one thing legible: how AI agents actually fail, in practice, using real public incident data.
Every case here is written the way a clinician would talk through a difficult case to a worried family member — plainly, carefully, and without exaggeration. The audience is anyone who is curious or concerned about AI agents and doesn’t have a technical background, plus the HR, operations, and leadership people who need to explain AI risk to their own teams without jargon. If a case note here can’t be followed by someone with no technical background, it hasn’t been written well enough yet.
An Important Disclaimer
This site is public-incident analysis, not a diagnosis of any specific company’s live systems. Cases are written from publicly reported, independently verifiable incidents (see The Taxonomy for sourcing). Vendors are not named unless the original public report already names them and doing so is material to the lesson — the point of every case is the failure pattern, not the company behind it.
The building, wards, and staff depicted in this site’s illustrations are also fictional. The AI Agentic Sanatorium is not a real hospital, clinic, or medical provider, and none of its imagery depicts an actual facility.
