Flag a public incident for review

Every submission is reviewed before publication, and not all submissions are published. Entries move through a moderation queue (Submitted, Under Review, Published, or Declined) and are checked against the sourcing bar described on The Taxonomy page before anything goes live.

Have a source link ready. That’s the only requirement to start; a description and a category guess help, but aren’t required.
