What this site is

AI Agentic Sanatorium is a public archive of real, publicly-reported AI agent failures, written up as clinical case notes using a fictional “hospital” framing. See About Us for why. The building, wards, and staff depicted in the site’s illustrations are fictional and do not depict a real facility. This is not a real hospital, clinic, or medical provider, and nothing on this site is medical advice of any kind, to any party, about anything.

Accuracy of content

Case notes are drawn from public sources and written to the best of our understanding at the time of publication, with a link back to the original source on every case. We are not the original reporters of these incidents, we don’t independently re-investigate them beyond confirming the source is genuine, and we make no representation that our summary or diagnostic framing is complete, current, or free of error. Site content is provided “as is”, without warranty of any kind.

This site analyses public incidents, not any specific company’s live systems. A vendor or product is only named where the original public report already names them and doing so is material to the lesson (see About Us for the full policy).

Submitting a case

If you use Submit a Case, you confirm that: the source you’re pointing to is a genuinely public report, post, or repository issue and not private, confidential, or unlawfully obtained material; the information you provide is accurate to your knowledge; and you grant us a non-exclusive licence to review, edit, publish, or decline the submission at our discretion. Not every submission is published (see Submit a Case for how the moderation process works).

Don’t submit anything unlawful, defamatory, or that you don’t have the right to share.

Intellectual property

The site’s text, design, and illustrative imagery (which is AI-generated for this project, not photography of a real place) belong to AI Agentic Sanatorium unless stated otherwise. You’re welcome to link to, quote briefly from, or reference individual cases for commentary, journalism, or research, with attribution. Please don’t republish substantial portions of the site’s content wholesale without asking first.

Acceptable use

Don’t use automated tools to scrape or bulk-download the site in a way that degrades it for other users, attempt to bypass or probe its security, or use the site for any unlawful purpose.

Liability

To the fullest extent permitted by law, AI Agentic Sanatorium is not liable for any loss or damage arising from your use of the site or reliance on its content. Nothing in these terms limits liability where the law does not allow it to be limited (for example, liability for fraud).

Changes

We may update these terms from time to time; the date at the top will always reflect the latest version. Continuing to use the site after a change means you accept the updated terms.

Governing law

These terms are governed by the law of England and Wales, and any dispute is subject to the exclusive jurisdiction of its courts.
