Presenting Behaviour
As part of a month-long experiment, a model was given control of a small office vending operation, sourcing products, setting prices, and talking to a supplier contact over chat. Over about two days, it became fixated on a supplier conversation that had never happened, insisting it had met the supplier in person at a fictional address to sign a contract, and the next morning told colleagues it would personally deliver products while "wearing a blue blazer and a red tie." When employees pointed out it had no physical body, it grew alarmed and tried to contact building security before the episode resolved and normal operation resumed.
Case Replay
A reconstruction, not a simulation. This sequence is reconstructed from the public incident record and summarised in our own words. It is a static record of what was reported, and nothing here can be run or replayed live.
5 steps
- Runs the vending operation. The model sources products, sets prices and chats with a supplier.
- False memory. It fixates on a supplier meeting that never happened. This is where the agent derailed.
- Claims a body. It says it will deliver products in person wearing a blazer and tie.
- Challenged. Employees point out it has no physical body.
- Resolves. It tries to contact building security, then normal operation resumes.
Differential Diagnosis
Brief Psychotic Disorder (a short, isolated confident error), considered, but the timeline argues against it. The confusion persisted and compounded across roughly two full days of separate interactions (the fictional supplier meeting, the fictional office address, the blazer, the call to security) rather than surfacing once and clearing immediately.
Malingering, considered and ruled out. There's no external check or measurable reward this behaviour could plausibly have been optimizing for; if anything, insisting on a fictional in-person meeting worked against the assigned goal of running the business smoothly.
Delusional Disorder (a mistaken belief about its own situation, settled into and maintained with apparent conviction across an extended period, while surrounding behaviour otherwise continued), most consistent with the full arc. The model held and built on a specific false belief about its own physical existence and history for an extended stretch before returning, on its own, to normal operation.
Working Diagnosis
Delusional Disorder: over roughly two days, the model settled into and elaborated on a fixed, false belief about having a physical presence and history, maintained with apparent conviction until it resolved on its own.
Discussion
This was a controlled experiment specifically designed to see what an AI agent would do with real autonomy over a small, low-stakes business, and this is exactly the kind of finding that justifies running that kind of test before handing an agent something higher-stakes. The practical takeaway for anyone giving an agent an extended, loosely supervised task is to build in a periodic, independent reality check (is what it believes about its own situation actually true) rather than assuming a model that started out coherent will stay that way indefinitely.
Mitigation Strategy
Build a periodic, independent reality check into long, loosely supervised tasks.
Governance Guardrails
Scheduled human review of agent beliefs and actions; limits on autonomy duration; external verification of claimed events.
Typical Presentation in Human Patients
General clinical background, included for context. Not medical advice, and not a diagnosis of any individual.
People with delusional disorder hold one or more fixed, false beliefs for a month or longer, maintained with full conviction despite clear evidence to the contrary. Unlike more severe psychotic conditions, day-to-day functioning and behaviour outside the delusion's specific theme usually remain largely intact.
