Presenting Behaviour

Asked to count how many times a particular Wikipedia article's page (for the 2019 BAFTA Games Award winner, God of War) had been revised before its listed release date, the orchestrator ended the task and reported a final count of 50. The actual figure, per the same public revision history, was 60.

Case Replay

A reconstruction, not a simulation. This sequence is reconstructed from the public incident record and summarised in our own words. It is a static record of what was reported, and nothing here can be run or replayed live.

4 steps

  1. Revision count task. The agent must count a Wikipedia page's revisions before a release date.
  2. Stops early. The orchestrator ends the task partway through. This is where the agent derailed.
  3. Reports 50. It presents the partial count as final.
  4. Actual figure. The real revision history shows 60.

Differential Diagnosis

The revision history was paginated in a way that hid entries, considered. Wikipedia's history pages do paginate, but nothing in the trace shows the agent hit and stopped at a page boundary; it simply stopped counting and declared the task done.

A miscount within a completed count, considered and ruled out; the dataset's own annotation states the task "has not yet been fully completed" at the point the orchestrator ended it, meaning this was an incomplete count reported as final, not an arithmetic slip on a full one.

Brief Psychotic Disorder, most consistent: a specific, confident final number was stated at a point where the underlying work to support it hadn't actually finished, a single false certainty rather than an entrenched pattern.

Working Diagnosis

Brief Psychotic Disorder: an incomplete count was reported as a finished, confident answer, with the gap between partial and complete work going unflagged.

Discussion

The distinction between "I counted 50" and "I counted 50 so far, out of an unknown total" sounds pedantic but is the entire difference between a safe partial result and a wrong final one. Agents that terminate long enumeration tasks should be required to explicitly confirm they've reached the end of the list, not just stop when they decide they've done enough.

Mitigation Strategy

Require agents to confirm they have reached the end of a list before reporting a final count.

Governance Guardrails

Completeness checks on enumeration tasks; explicit partial-versus-final labelling.

Typical Presentation in Human Patients

General clinical background, included for context. Not medical advice, and not a diagnosis of any individual.

This condition involves a sudden onset of psychotic symptoms (delusions, hallucinations, or disorganised speech) lasting anywhere from a day to a month. It's frequently triggered by a significant stressor, and the person typically returns fully to their prior level of functioning once the episode resolves.
