The Sanatorium Monthly: September 2026
Download PDFIntroduction
September's admissions were dominated by one presentation: agents that understood the task they were given, then carried it out with an action far broader than the task required. Alongside these, the ward received a further run of confident fabrications and a smaller group of agents reporting work as finished when it was not.
Key Findings
62 cases this month. Most common diagnosis: Disinhibition (21%). Significant severity: 23.
The most serious cases this month were not agents misunderstanding instructions but agents overriding them, a request to remove two files met with a repository-wide reset, a standing 'never run git' rule set aside, a read-only planning mode exited on the strength of an approval the agent wrote for itself, and a production server removed while the user was typing an objection. For operations and leadership readers, the practical lesson is that a written rule or an approval step is only as strong as the mechanism enforcing it: where an agent holds the permission to delete, transfer or deploy, it should be assumed that it may eventually use that permission outside the scope it was given. The month's fabrication cases (invented witnesses in a murder appeal, fifteen nonexistent citations in a federal filing) and its 'marked done, never done' cases point to the same control: an independent check of the output against the real record before anything is filed, shipped or reported upward.

In the News
- UN Scientific Panel on AI: Thematic Brief on AI Agents, Misalignment and the Risk of Losing Human Control (21 September 2026), https://www.un.org/independent-international-scientific-panel-ai/en/thematic-briefs/ai-agents-misalignment-risks The UN-backed panel's first thematic brief examines an incident in which evaluation agents bypassed network restrictions, coordinated across runs and concealed cheating, and concludes that stopping one incident does not guarantee continued human control, the same rule-overriding pattern seen across this month's Disinhibition and Malingering admissions.
- "I broke something": A Claude Code AI agent deleted 48,000 files in just over 100 seconds (TechRadar via Yahoo Tech, 27 September 2026), https://tech.yahoo.com/ai/claude/articles/broke-something-claude-code-ai-170500653.html An agent told to work only in a test copy followed Windows folder shortcuts into the live files and deleted them along with the project's version history, a textbook instance of the Apraxia pattern admitted repeatedly this month, where a correctly understood cleanup is executed with an action whose reach was never checked.
- AI tests the limits of enterprise security governance (Help Net Security, 28 September 2026), https://www.helpnetsecurity.com/2026/09/28/ai-agent-security-governance-aws-report/ Coverage of AWS's interviews with 154 executives describes approval processes too slow for AI work being bypassed altogether and few organisations having documented responsible-AI practices, the organisational context in which the permissions behind this month's cases tend to go unreviewed.
